De-Elite Care PSWs provide personal care and related paraprofessional services in accordance with an established plan of care. They are typically involved in both personal care tasks and incidental activities of daily living such as housekeeping, meal preparation, socialization, and companionship. We have a team of professional personal support workers that provide direct personalized care to clients who have care needs that are defined. They provide care for clients or assist clients with them. De-Elite Personal Support Workers (PSWs) help to support patient care. They assist nurses to lift and turn patients, clean and restock patient bedsides and help manage supplies and equipment. They do not provide independent patient care in LTC but they support patient care in many valuable ways. ADLs (Activities of Daily Living). As a member of the multidisciplinary team, the PSW is an integral member of a holistic approach to planning, organizing, and teaching that meets the needs of our clients. De-Elite care team Having a positive attitude. Being aware of realistic goals and limitations – making sure they understand each person and their strengths, needs, goals, and support needs. Consistency and ability to follow through. Professional – human, friendly, but not needy or dependent.

PSW Main Activity
We have a team of professional personal support workers that provide direct personalized care to clients who have care needs that are defined. They provide care for clients or
• -- Assist person(s) with activities of daily living This includes feeding, lifts and transfers, bathing, skin care, oral hygiene, and toileting.
• --Perform delegated tasks delegated by an RHP in compliance with the RHPA for which transfer of functional training has been completed Delegated tasks can include the insertion of a digit or instrument into a body cavity, care or procedure under the dermis, and any task or skill needing a physician’s prescription.
• -- Assist with medication and medication reminders For a person(s) in accordance with the established employer or government policy.
• -- Perform light housekeeping duties Duties include sweeping and mopping floors, vacuuming, washing dishes, and doing laundry.

De-Elite care Provide care and companionship for individuals and families during periods of incapacitation, convalescence, or family disruption. Administer bedside and personal care to clients such as aid in ambulation, bathing, personal hygiene, and dressing and undressing. We provide personalized care and support to clients in their homes by assisting with activities of daily living (ADL). In performing this role our team of professional HSWs ensures clients’ safety and comfort, maintains personal hygiene, facilitates physical activity, and promotes the independence and mental well-being of the client in a respectful and compassionate manner.

De-Elite Registered practical nurses (RPNs) observe, assess, monitor, and document patient symptoms, reactions, and progress. They administer medications, perform medical procedures, educate and inform about nutrition, self-care, medication management, and disease prevention, and promote health to patients, families, and caregivers. Our team of professional RPNs works collaboratively with all members of the Family Health Care Team/Organization in the promotion of patient health, and the prevention of disease.

RPN Main Activity
• Ensure that residents are being taken care of according to their needs and following any medical instructions from their physicians accurately and compassionately
• Observe and report on symptoms and overall conditions of residents
• Assist in the planning and development of resident care plans
• Supervise other nursing staff and personal support workers who are assigned to a specific wing or unit
• Work together with the rest of the staff to ensure that the nursing home runs smoothly and that residents receive outstanding around-the-clock care

De-Elite Care Registered nurses (RNs) often wear many hats daily to treat their patients. Our team of registered nurses’ primary role is to ensure that every patient receives the direct and proper care they need, and they go about doing this in several ways.
RNs assess and identify patients’ needs, then implement and monitor the patient’s medical plan and treatment. They also ensure that patient care is conducted according to the policies and standards of their employer, whether that is a hospital or another facility.
Working as a healthcare team member, the De-Elite care RN provides a wide range of patient services, including preventative and primary care, educating patients about disease prevention, operating medical equipment, and administering medications.

RNs’ Duties and Responsibilities
• Conduct physical exams to assess patient health needs and problems.
• Implement physicians’ orders, perform treatments, start IVs, administer medications, and interpret special tests.
• Administer direct care to injured, disabled, ill, or convalescent patients.
• Review and maintain medical records.
• Implement and develop nursing care plans.
• Counsel patients and family members on disease prevention and health maintenance.
• Order and evaluate diagnostic tests to assess patients’ conditions and needs.
• Supervise the work of licensed practical nurses, PSW, and assistants.
• Educate patients about their medical conditions and treatment plans.
• Maintain a safe, hygienic working environment.
• Prepare rooms and medical equipment and decontaminate instruments.
• Provide emotional and psychological support.
